What the study found
Therapeutic plasma exchange (TPE, a procedure that removes and replaces plasma) in acute immune-mediated thrombotic thrombocytopenic purpura (iTTP) was usually tolerated, but complications were frequent. The most common problems were minor catheter-related bleeding and allergic reactions, while some patients had major catheter-related venous thrombosis or infection.
Why the authors say this matters
The authors state that TPE is the cornerstone of treatment for iTTP, but that it is not risk-free and there is scarce evidence about its safety in this setting. The findings indicate that complications should be considered during treatment, especially those related to the central venous catheter used to start TPE.
What the researchers tested
The researchers conducted a cross-sectional study of patients hospitalized for an acute iTTP episode at a hospital in Milan between January 2016 and April 2022. They enrolled episodes in which patients underwent central venous catheter placement to begin TPE and described TPE-related clinical complications.
What worked and what didn't
Thirty-three iTTP episodes from 32 patients were included. Twenty-one episodes had at least one major TPE-related complication, and 18 had at least one catheter-related complication; 9 had a major catheter-related complication.
Major catheter-related complications included 5 venous thromboses requiring heparin treatment and catheter removal, and 3 infections requiring systemic antibiotics and catheter removal. The 19 catheter-related bleeds were all minor and needed only local hemostatic medication. Among major procedure-related complications, allergic reactions were most common (16 episodes) and clinical hypocalcemia occurred in 8 episodes.
Episodes with catheter-related venous thrombosis or infection had longer hospital stays, and infection was also associated with a higher number of TPE procedures.
What to keep in mind
This was a single-center cross-sectional study with a relatively small number of episodes. The abstract does not describe comparison groups beyond the complication categories, and it does not provide details on long-term outcomes or prevention strategies.
